steps to complete interagency referral:
ce the member has returned the signed Interagency form, you would email it to the primary and second contact for that agency along with the pre-screening form. Their email can be found under the "Contact for referrals" form. Please also CC your direct Family Assistance Manager. In the subject line write "HMIS # Sacred Heart HPS Inter-agency referral". In the body, include the reasoning for the Interagency Referral (i.e., capacity, requested transfer, etc.). Write any other important details that would be beneficial for the agency to know such as if they have received a late notice, member's use of technology, state application, and landlord pressure.
