As the OT, you have given the OTA a treatment plan to apply a hot pack to the client's lower back 20 minutes before starting the IADL session where the client completes laundry while standing. The OTA goes to prepare the session, thinking that the client is in the ADL gym, by taking out the hot pack and placing it on the table 10 minutes prior to the client actually showing up. Once the client finally shows 10 minutes later, the OTA places the hot pack on the client for 20 minutes. Afterwards, not more than halfway through the laundry session, the client states "I'm sorry but I cannot continue; my back is in pain".
