A nurse marks the patient an 8/10 for anxiety in the clinical scales for new onset of anxiety.
New anxiety therapies and/or medications now ordered.

When a nurse and/or sw indicate a patient has screened positive for moderate or severe anxiety, they must activate this
This is where the nurse or SW would assess any patient who has a diagnosis of anxiety or has any new issue with anxiety
What is the anxiety screening within the assessment?
The SW screens a patient positive for severe anxiety and initiates a care plan and discusses this finding at ITD
The nurse has not completed any boxes on the anxiety screening in his/her assessment
When the patient has screened positive for moderate or severe anxiety and they have already had an anxiety care plan activated, they should also have this updated
What is a diagnosis for anxiety?
SW's: This is where I would indicate how to best care for a patient's anxiety.
What is a SW anxiety care plan?
The patient has a diagnosis for anxiety.
Nurse and SW do not complete any anxiety screenings within their assessment
What is anxiety screenings should be completed for all patients with a diagnosis of anxiety?
Nurses (&CNA's) This is where I would document and also find direction on how to care for my patient with anxiety
