Student breaks eye glasses. Student breaks arm. This student must still test. They can receive accommodations if needed, but they are required to test. An illness is NOT an medical emergency. Students who are too ill to come to school and may not return during the testing window will have to test at home.

If a teacher is not present in the room, what is the proper protocol for securing test materials?
What is under two locks? Classrooms are not equipped to secure test materials. A teacher should always be present and able to view materials at all time. If a teacher needs to leave the room, they should call Mrs. Barron.

This is the order which should be followed for test administration?
What is in the order of the test booklet? This means all third graders should be taking the same portion of the test at the same time and must go in order of the subject areas in the booklet. Test administrators cannot jump around in the test booklet.

These resources are approved for all content areas of the K-Prep.
What are #2 pencils, blank or lined scratch paper, and erasers. Scratch paper must be turned into the BAC with all test materials and destroyed daily. Scratch paper may NOT be used for fifth grade on demand writing. All pre writing must be done in the test booklet. A writers reference sheet will be provided and must be returned.

Give two examples of extraordinary circumstances according to K-Prep?
Recent traumatic experience Child placed in protective custody during testing window. Depending on the situation, these students may still be required to test. BAC will contact DAC who may communicate with the state. We had several extraordinary circumstances last year.

Name the most significant change to the seating chart requirements for the 2013 assessment window.
One seating chart can be used for the entire testing window. Asterisks should be used to signify when a student is absent or has to leave the room. (i.e.: Kaet absent on 5/2.)

This must come between every test section (even when testing a student individually).
What is a break? It can be a short break (10 minutes) or a long break (over lunch or over night.) During breaks longer than 10 minutes or when all individuals leave the room, materials must be secured by the BAC first. (ie: On Thursday, May 2, fifth graders cannot leave for lunch until their test booklets have been secured.)

Who receives extended time?
What are students who have extended time listed in their IEP or PSP? Last year, all students received extended time because the test was in it's preliminary year. This year, testing times have been developed to included any extra time needed. The only students who received extended time this year are those with it listed on their IEPs or PSPs.

A student gets a nose bleed or vomits on a test booklet. What is the procedure?
Answers must be transferred to another test booklet (of matching form). A witness must be involved with all answer transfers. The contaminated booklet must be saved, then securely destroyed once the DAC has given permission. For your own protection, let the BAC transfer all answers. Do not do this yourself or even with another teacher present. Do not destroy any test materials. Let the BAC handle that.

5 pieces of information: Date and time of incident All individuals involved (staff, students, parents, etc.) Content area and test part affected (Math, part 1 B) Specific content questions (if appropriate--do NOT include test questions in report, just the numbers!!!) Record what you learned 4 documents: Testing allegations reporting form Notarized document on school letter head, written and signed by each individual involved (1 form with multiple signatures is NOT acceptable.) Testing Schedule Seating Chart with Room Numbers (BAC must provide additional documents including: signed admin codes and proper test administration forms for proctor, non disclosure agreement forms signed by all involved, student name and ID, etc.)

Give 3 examples of when a student can participate in make up exams.
Once the child has returned to school and a new testing section has begun (ie: a third grader is tardy to school. She arrives during Math, part A. She can join in with her class during Math, part B. She will make up Math, part A in the afternoon.) After the normal testing schedule has completed for the day. (A 4th grader is struggling to behave appropriately during scheduled test time. He is removed from the room. Once the normal testing has concluded, he can make up his assessment with an accomodater one on one in the afternoon.) During one of the four scheduled make up days following testing (May 9-10, May 13-14) This is for students who miss several days of testing due to illness, etc. The recommendation is for a child to make up the missed portion of the test as soon as possibly.
