3 Part question: An Enrollment Appeal is intended for members who had special circumstances preventing them from enrolling in benefits within their event window. An appeal is required to be submitted with what requirements/and how can EE send in appeal / what important information that we also need to provide.

  • The employee's name
  • Employee ID number
  • Written request stating the reason for missing the enrollment deadline and a reason for the appeal
  • Employee's signature and date
  • Any other supporting documentation

The employee can send the appeal in one of the following ways:

The FedEx Appeals Committee will review appeals to make sure plan guidelines were followed. If followed correctly, denials will be upheld.
