The quotation mark should be after the quote ends, not after the page number... and the P should be lowercase.

List by last names and initials; commas separate author names. After the sixth author's name, use an ellipses in place of the author names.

NO! Do not change the title. You must cite it exactly the way it was published.
Should personal communications be cited in a References list? Why or why not?
Because personal communications (letters, memos, interviews) do not provide recoverable data, they are not included in the reference list. Cite personal communications in the body of the text only.
