Mitigation activities generally occur before an emergency and are designed to reduce the risk of and potential damage from an emergency so mounting bookcases and shelves and checking the smoke alarms are mitigation activities.
How do surveyors assess if Edgewood is upholding TJC standards?
Surveyors will utilize the "tracer" method to track a client through the program(s) in the sequence they receive care. During this process, they review client records, talk with staff members, review personnel records and conduct walk throughs of the office site.

How do we protect client privacy when it comes to documentation storage and information sharing? (Name 5)
We are aware of our surroundings when discussing client information
We only view PHI necessary to do out job
We pick up our printed documents promptly from the printer and have a secured print option.
We store PHI in a locked cabinet, room and building "rule of three".
We have shredding bins
We have passwords to enter our systems
We encrypt emails
We don't store PHI personal devices
When we are in the field, we carry a lock box with us and ensure the documents in our possession the entire time.

1.Family members/natural supports: They are involved through family conferences, and by the team offering to connect and provide education to families about the services we provide and ways we can include them in treatment.
2. External Providers: They are involved by participating in family conferences, warm hand off meetings
Who is responsible for the immediate care and reunification of youth that are onsite with or without their families?
Program staff is responsible for the reunification and care of youth that are onsite with their families or responsible for the care of youth in the absence of families.

What does good discharge planning consist of when a client is graduating or transitioning out of the TAY FSP program?
Transition and graduation is discussed with the client from the very beginning of treatment.
Adjunct members gradually taper off client's case.
Referrals/resources i.e. suicide prevention and community resource information is given to the client
Discharge instructions i.e. medication and treatment recommendations are given to the client.
Information re. client treatment/service provided is exchanged with next provider (with release).
Warm hand off meetings are typically scheduled.
What is trauma informed care and how do we incorporate this into treatment?
Trauma-informed care shifts the focus from “What’s wrong with you?” to “What happened to you?” A trauma-informed approach to care acknowledges that health care organizations and care teams need to have a complete picture of a patient’s life situation — past and present — in order to provide effective health care services with a healing orientation
We incorporate this by doing the following:
-Staff are trained in trauma informed practices and interventions, as well as the impact of trauma on clients, and staff susceptibility to vicarious trauma.
-Clinicians document clearly document the course of trauma treatment
-Staff engaged in debriefing protocols post crisis situations
-Staff participate and receive ongoing supervision to mitigate vicarious trauma and aid in informing clients of treatment
What is the most important Infection Control (IC) activity?
Handwashing. Also, side note - Edgewood recommends everyone gets a flu shot.
