All providers wear badges with their name and credentialing. The providers also introduce themselves when they enter the room.

We have Stratus, that is located in patient relations on the first floor. ER also has a machine. Call house at 80909 if you can’t find a machine.

We screen all patients during intake using PHQ9. if score is higher than 10 or they score positive #9 then we contact Rachel and put it in the teams MFT chat.

How often do you perform medication reconciliation?

We do this at every visit. We ask the patient to always bring an updated medication list for each visit.

Rescue: Move anyone in immediate danger to a safe area.

How are patients and families informed of their rights and responsibilities?

These are mounted in the WC lobby, on the front and back of the door next to the front desk. This information is also available through the Compliance department (Tammy Middleton is the contact for this resource)

Do you have a procedure in place to screen for domestic violence or abuse?

We screen ALL patients privately for abuse at EVERY visit.

What would you do if you learned of a fire in another part of the building?

If the fire is located in dental, we do nothing. If the fire is located on inpatient side, we do nothing. If the fire is located in the outpatient pharmacy building, we do nothing.

We do nothing because there is a firewall.

Where can you find documentation that an interpreter was used? 

There is a log on the stratus machine, and nursing places a note in EHR.

When do you label medications?

• Label a medication whenever you are not going to immediately administer the dose to the patient. Immediate administration means preparing the medication and taking it directly to the patient with no break in the process. This should occur even if only one medication is being used.
