How do you know when you need to make a referral for further physical health assessment?
Well, if the client’s last physical exam is more than one year old or if the client is reporting an untreated physical issue or pain that is interfering with daily life, I would make a referral to physical health practitioner. I can also contact our Medical Services Department if I ever have any questions. They are super helpful!

Here at SFA we are all about communication... We hold team meetings and/or phone calls with all the providers working on the case to make sure that we aren’t duplicating efforts and are all working towards the same goals – the client’s wellness and improvements in the client’s life!

When something unexpected and very negative happens with one of your clients, how do you alert the right people? How do you report these kinds of incidents?
Well, the first thing that I do, is notify my supervisor and then, as soon as possible (w/in 24 hrs.) or before I leave for home, I complete an Incident Report on the system. This report is then distributed to everyone involved in the case.

We involve our clients (and families when appropriate) in the treatment planning process. As the treatment plan really is a contract with the client, we want to send the message that the client is at the center of its development and all the goals must reflect what the client wants to work on!
