One's hair is dirty and unkempt.

If the client is able to look up emergency numbers without assistance and cues, the therapist would use this score.

What is independent?

If the client's spouse or partner is responsible for all money management tasks, the therapist would give the client this score.

What is a see note?

The client uses only walking for transportation, does not leave the house, or all methods stated require the participation of another person or another person's property?

What is needs assistance?

For item "plans for future employment," the therapist would give this score if the client is retired.
