Earned Time (ET) is an inclusive paid time-off benefit program intended for use in connection with vacations, holidays, non-work-related injuries and illnesses of five (5) days or less, personal business, bereavement, family care, continuing education and other needs which may require time off from work.

What is FMLA and who is eligible to use it

FMLA is the Family and Medical Leave Act. An employee who has been employed by the Health Center for at least one (1) year and who has worked at least 1250 hours over the previous 12 months is eligible for leave under the FMLA. (Section 4.9)

Based on the the dress code what are three items of clothing or accessories that an employee is not allowed to wear or have on during work

·Skin-tight leg wear, unless worn with a covering garment not shorter than 1-2” above the knee.

· Jeans - Employees with no patient or public interaction may wear jeans during the work week. Staff that are patient facing may wear jeans on Fridays only.  All jeans should be in good condition, not ripped, sagging, or patched.

Generally how long is the probationary period at LCHC and is there a possibility that this period could be extened 

Section 6.12

The first three (3) months of employment for all employees are probationary months. A Supervisor may recommend that a new employee’s employment be terminated at any time during the three (3) month probationary period if the overall evaluation of the person and the position warrants such action. 

At the end of the three (3) months, the employee’s performance will be appraised.  At the appraisal, the supervisor will inform the employee that the employee will become a regular employee, that the probationary period is being extended, or that the Health Center no longer needs the employee’s services.

How do you report an absence?


 

You must notify your Department Supervisor as the supervisor instructs, but in no case later than 60 minutes prior to the scheduled starting work time. (Section 3.3)

What is Extended Sick Leave(ESL) and when can it be used?

Extended Sick Leave (ESL) is a paid time off bank intended for illnesses.   ESL is used only for illnesses and injuries which exceed forty (40) hours for full time employees, or hours equal to one regularly scheduled work week for part-time employees. Employees will use Earned Time before accessing ESL.  (Section 4.6)

Employees may take up to 26 weeks of Military Caregiver leave during a single 12-month period. For all other types of FMLA leave, each employee may be granted leave for a period up to twelve weeks (during any twelve-month period).(Section 4.9)

What is a general leave of absence? 

A general leave of absence is when an employee requests for leave does not meet the qualifications for FMLA leave, or other types of leave, or which exceeds the twelve-week period of FMLA leave , may be considered for a General Leave of Absence without pay.  To be eligible, an employee must have completed at least one year of continuous service from their date of hire.(Section 4.12)

Examples of unauthorized use include 

1.Unauthorized attempts to break into any computer belonging to LCHC or any other organization.

2.Using LCHC time and resources for personal gain.  Personal gain includes job searches through online bulletin boards, performing online trading and excessively utilizing the Internet for non-job-related functions.

3.Using LCHC time and resources for downloading or playing games, downloading non-business-related graphics or cartoons, excessively “surfing” the Internet, or reading non-business-related news groups.

4.Posting or forwarding a chain letter utilizing the Internet or the Health Center’s computer and e-mail system.

5.Visiting inappropriate Internet sites (e.g., pornographic, chatting, non-business-related shopping, and gaming).

What happens if you fail report unexcused absences to your supervisor? 

If you fail to call your supervisor for three (3) days of unexplained absence, you will be deemed to have resigned voluntarily.  Employee attendance is part of the performance appraisal (Section 3.3)

What is Mass earned sick Time law, how often does it accrue, and who is eligible to use it? 

Employee that are not eligible for ET can use Mass earned sick time law. Sick time accrues at the rate of one hour for every thirty (30) hours worked per benefit year, up to a maximum of forty (40) hours. (Section 4.8)

What are the steps to request a leave of absence and how far in advance must you request this leave of absence? 

An employee seeking a leave of absence must submit a written request for leave to his or her supervisor.  The request for leave must include:

·         Name of employee   

·         Name of supervisor

·         Date of request

·         Reason for leave (please provide specifics)

·         Anticipated timing and duration of leave

·         Signature of employee

 Employees must provide 30 days advance notice of the need to take a leave of absence when the need is foreseeable (Section 4.14)
