- Month and year account was opened
- Amount of any check posted in the last 30 days
- Amount and date of ACH, ATM or debit transaction in the last 30 days
- First six and last four digits of open and active debit card (if not given in Step 3)
- Name of a beneficiary on any of customer's accounts
- Joint account owner's date of birth including month, day and year
- If customer answered incorrectly twice: Go to Step 5
- Tell customer transaction can't be performed over the phone in the branch but they can do one of the following:
