This is the person that helps you find services when you leave Shen. You will call, email or text them with questions that you have so they can give you answers and support you.
Care Manager
You choose a program and during the day you go with your program to travel around the community to learn new things and volunteer. You take a van or bus and go somewhere new each day.
WOW- Without Walls
An organization that offers different sports so you can stay active and spend time with friends. They offer coaches and have teams for things like bowling, bocce, track and other sports.
Special Olympics
This is the document that everyone is talking about at your Life Plan Meetings. It has important information like the name of your care manager and the agencies you will work with. It also has all of your goals, interests, skills and things you need help with.
Life Plan
This is a service you can pay for and go to weekly exercise training sessions. It is a disability friendly gym.
St. Christopher's
This is the person that helps you with challenges on the job. When you graduate, this person won’t be paid for by Shen, they will be paid for by your SEMP agency.
Job Coach
This is how you get job coaching when you leave Shen. You choose the agency that supports you at your job. Some of the agencies are Living Resources, Life Song, Wildwood and Saratoga Bridges.
SEMP- Supported Employment
This is a free service that you can go to on the weekend with family, friends or a comm. hab. worker to spend time with other people with disabilities and socialize. It is run out of St. Edwards Church but is NOT related to the church.
Our House
This is not paid but it allows you to get job experience. You try out different jobs and get work skills.
Pre-Voc
These programs give you time to work on your independence and learn about things you are interested in like cooking, organization and other things that can help you as an adult.
College Experience Programs
This is a person that can help you access the community. They can bring you to things like Our House and St. Christopher's or they can help you run errands and work towards your goals.
Comm Hab Worker
This stands for Office for People With Developmental Disabilities. It is responsible for coordinating services for New Yorkers with developmental disabilities
OPWDD- Office for People with Developmental Disabilities
These are evening classes that you pay for and attend based on your interests. They offer things like jewelry making, karaoke, ASL, and cooking classes.
Wildwood Lifelong Learning Classes
This is when you get paid to work through New York State and you have a job coach to help you explore different types of jobs.
ETP- Employment Training Program
This gives you the chance to choose your own services so you can live the life you want. When you self-direct your services, you have increased flexibility to choose the right supports for you, the staff you want to work with and a schedule that works best for you.
Self-Direction
