2 Educators must be present for medication to be administered.

 One Educator must confirm that the medication belongs to the child listed on the medication form, the second educator will administer the medication once verified by both educators that the name on the medication and the child receiving medication match.


2 Educators must document and initial the following information on the medication consent form:

o Time of administration

o Dosage

o Name of medication

o Initials of the Educator who administered the medication
