Social Anxiety

Collateral Information
fear or anxiety about two (or more) of the following situations: public transportation, open spaces, enclosed spaces, standing in line, outside of home alone.
Agoraphobia
Two (or more) that have occurred for most of a one month period: delusions, hallucinations, disorganized speech, etc. Level of functioning below level prior to onset, signs of disturbance persist for 6 months
schizophrenia
Depressed mood, significant weight loss/gain, fatigue, diminished ability to think, insomnia or hypersomnia nearly every day for the - all for the same two week period

Treatment plan
