What is a Section 12B in the state of Massachusetts?

It is a legal document signed by a psychiatrist temporarily suspending a patients rights for 72 hours for psychiatric evaluation. (Business days)  

Reasons include: the patient being a hard to themselves, a harm to others, or not being able to take care of themselves

What is the goal when working with someone who was admitted after a suicide attempt?

Allow they to discuss the event if they need to, set boundaries so other patients are not triggered. Remind patients they are safe here and help them find activities to use as distraction coping skill

You are not allowed to accept gifts. If someone colors you something or makes a craft, put it in the shared office.

What is the KELS, what is it used for? 

Kohlman Evaluation of Living Skills (KELS). Observation and interview based assessment. This is an assessment of safety, and is typically used for to assess safety.

To gain insight into our clients hx (history) and remain client centered

What does it mean if a patient signs a conditional voluntary? 

All patients are admitted to a psychiatric hospital on a section 12B. During the admission process a patient is asked if they want to sign in voluntarily. 

This means that they agree with their admission and want to work with the doctor until they are no longer a harm to themselves, a harm to others, or able to take care of themselves. 


If they do not sign this, they are brought to court.

What is the goal when working with someone who has MDD (major depressive disorder) 

Encourage patient to be out of there room, encourage activities that are familiar and important or that they once found familiar and important (remember with anhedonia)

A patient approaches you in the hallway and is clearly delusional, how do you respond?

Do not acknowledge that the delusion is real, but do not disprove it. Distract them with an activity and end the interaction. A 1:1 session is not needed for this patient at this time

What is a 3 day note? 

After a patient signs in voluntarily, they have the right to sign a 3 day note. 

This means they will give the psychiatrist 3 BUISSNESS days (no holidays or weekends) to observe the patient. At this point the psychiatrist either discharges the patient, or they are brought to court.

What is the goal when working with a someone who is experiencing mania?

Set limits. Set limits with behavior, choices, and materials. Make sure you stick to what you say. "If this...then that..."

What is staff splitting?

When patients try to pit staff against each other, "your the only good group leader?" "Nurse Rachet says you don't know what your doing"

What is the ACL, what is it used for? 

The Allen Cognitive Level Screen (ACLS), also known as the leather lacing tool and the leather lacing test, gives you a quick measure of a person's global cognitive processing capacities, learning potential, and performance and problem-solving abilities. 

This is often used for aftercare, or to determine appropriate groups

If someone is experiencing AH or VH (auditory or visual hallucinations) what areas of occupation and performance skills may be impacted?

ADLs, IADLs, Leisure, completing tasks. Treatment should still continue and 1:1 supervision is only needed if the patient is a harm to themselves, a harm to others, or they can not take care of themselves.

What is a Rogers?

A Rogers is when a patient is court ordered to take meds IN THE COMMUNITY. Rogers are not legally enforces in when a patient is admitted to an in patient unit.

What is the goal when working with someone who is experiencing grief? 

Allow people to talk about the person they lost, but do not allow them to drown in their grief. Help them channel it into a create project.

Cognitive functioning in the elderly. This is primarily used in geriatric behavioral health, and it can help rule out organic reasons for behavioral health symptoms

What is a section 35? 


A section 35 is when someone is involuntarily committed for substance abuse. 

The biggest difference is section 35 lasts for 30 days, not 3

What is the goal when working with someone who is experiencing paranoia?

Make sure that you explained the expectations of the group, or intervention. Establishing rules is also helpful. Make sure everyone in the group can be seen, and clients, and staff are not engaging in side conversations.

You enter the the dining room and see a patient with their back to you, you approach the patient from the side and you see the patient is scratching their arm with a plastic fork prong. What do you do?

If the cuts remain superficial, engage in a conversation. "Why are you doing this?" "Can I please have the fork" "What do you need from me?" 

If they are deep, you call for staff immediately. This patient may need a 1:1 if it continues.

What is the MoCA, what is used for? 

Montreal Cognitive Assessment (MoCA) is a quick cognitive assessment. Often used to assess for Mild impairment and Alzheimer's disease.

This is typically used as a screen to see if further neuro psych testing is indicated

You introduce yourself yourself to a new staff member in the employee dining room. This employee is new to the field of behavior health. This staff member is surprised that an OT works here, "I thought you helped fix hips and knees, and sometimes worked at schools" How do you explain your job and your scope of practice
