Today, the U.S. correctional system has 10 times the number of mentally ill individuals than any state psychiatric hospital (Stringer, 2019).

There are now more seriously, and persistently, mentally ill individuals imprisoned in correctional institutions today than in all U.S. state psychiatric hospitals combined, and the number of state prisoners with mental illnesses of varying degrees and severity is estimated to be around 1.25 million (Nishar, 2020).

  • Broad Personality Disorders: Some studies report high numbers when any type of personality disorder is considered. For instance, one meta-analysis found 65% of male prisoners had a personality disorder, while another suggests that over 60% of incarcerated males are at risk.

    • Antisocial Personality Disorder (ASPD): Studies focusing on ASPD show varying rates. One study reported that nearly one-third of incarcerated men had ASPD, while another found 47% of male prisoners with a personality disorder had ASPD. 

    • Cluster B Disorders: Research has shown that approximately 29% of incarcerated men had a Cluster B personality disorder.
