If a parent is having phone issues, calling and texting may not work, so other alternatives must be used prior to closing a case.

Quarterly reviews must be done within 80-90 day time frames and are stated in the IPOS.  This is a good time to review progress from clinician and family perspectives.  Give a combination of quotes and ensure there is an update for ALL objectives.  Complete CAFAS to show progress.

Letting families know at the first appointment that we are planning for success, what that looks like, and services are not forever are ways of developing rapport and engaging with families.  Termination begins at the first session.

BPSA Update

When there is an unexpected change in any of these areas, a BPSA update needs to be completed instead of waiting for the annual update.

  • Psychiatric symptoms – e.g., new symptoms, symptoms that continue uncontrolled, substantial change in severity
  • Living arrangement – i.e. a major change in status, e.g. between dependent and independent settings, or change in homelessness status
  • Medical status – e.g., major illness, deterioration in medical functioning due to progression of an existing illness
  • Family / social network – e.g., death of a significant other, new child, loss of a major support
  • Criminal justice involvement – e.g., no involvement vs. on probation
  • Guardianship, conservatorship, or status as a dependent minor
  • Risk to self or others – significant increase or decrease
  • Cognitive ability
  • Job or school enrollment or performance

Natural supports include parents, friends,  relatives, volunteers, church members that assist in meeting the client/family needs and should be included in the IPOS.
