what do you say if an owner admits to NOT wanting any extractions, JUST a cleaning?

if the doctor recommends extractions, they should be open minded to helping their pet and let us use our judgment to prevent infection/pain. if a pet knowingly needs extractions, we will not perform just a cleaning.
