SS cannot administer any medicine unless deemed necessary by school nurse. Ensure that you document any medication administered during QUEST hours; provide school nurse with details/notate where required by school nurse as well as a parent communication log provided to parent at pick-up.

Notify Julissa- she will call Bruno's parents to check if their student will continue attending QUEST. This will also ensure she does not send out suspended account notifications to SS and school admin if student has withdrawn or will withdraw.

2-4 weeks.

Is the account holder financially responsible for anything after they have officially submitted a withdraw request in writing?
Yes, account holder is responsible for the 2 week period following written notification.
Mom tells you that Jayden has an IEP for dyslexia and ADHD, what should you do with that information?
Contact the school counselor and the student's teacher for any required accommodations currently being made for Jayden. QUEST will explore all avenues possible to understand and meet required accommodations.

While on the playground Sunny and Diego fell off the slide causing Diego to land on Sunny's leg, how do you handle the situation?
Instructor must start off by assessing the injury(ies), notify SS immediately, investigate how they fell off of the slide and document the incident/accident using the QUEST Incident/Accident Form.
SS must fill out the parent communication log prior to student(s) being picked up in order to facilitate the conversation with parent regarding the incident. Depending on severity of incident/injury, primary account holder should contacted first; if unsuccessful continue down the approved pick-up contact list. SS should scan both reports to their Program Coordinator prior to leaving for the evening.
Dad comes to your site to pick up their student but does not have a PIN, what do you do?
Log onto EZ Childtrack, check to see if he is on the approved pick-up list. If he is on the approved pick-up list, verify his DL number then create a PIN so Dad can use it right away. This way we have documentation of who picked up along with the time.
How many family engagement nights are you required to have each year?
At least two, one each semester.
Noah is a part of the school choir and every Wednesday his music teacher sends him to QUEST at 4:30PM, how do you handle this situation?
Parent must notify SS in writing of Noah's choir practice and his music teacher must walk him to QUEST to check him in. Noah cannot attend QUEST first then go to choir practice then come back to QUEST, he must stay with this teacher directly after school.

Student cannot be released to Uncle. SS must call Mom from the QUEST line to verify what Uncle has said; Mom must the add Uncle to EZ Childtrack and assign him a PIN before Jasmine is released to Uncle.
Mom calls and tells you she will not be able to make it by 6:30PM and Ariel can walk home, what do you do?
Inform Mom that Ariel cannot walk home and she will have to send an authorized adult to pick her up.
