F557 Dignity - Am I allowed to leave my PCC Screen up as I pass my meds?
No. Screens should be minimized for resident dignity and HIPPA compliance. Med carts and Treatment Carts should be locked each time you leave them unattended.
F803 Menus - If a resident has an altered diet do I still have to serve them whats on the menu?
Yes. We cannot substitute what is on the menu just because it has to be altered. Extension menus must be used and receipts for Altering Diet.
F925 Pest Control - How often should the rooms be cleaned?
Cleaning should occur in every resident room daily. This includes bedroom, under bed, wipeable surfaces and Bathroom.
F557 Dignity - Is a Catheter Bag need to be covered?
Yes. At all times. In rooms and in common areas. If cath bags are unavailable please contact Yoli for ordering.
F580 Notifications - Who should I notify if a resident is sent out to the hospital?
Resident's responsible party or Emergency Contact should be notified by the Nurse. Bed Hold should be given, explained and documented at time of transfer. Responsible party or Emergency contact should be notified of any incident/change in orders/change in condition/transfer and documentation should be completed.
F803 Menus - Do Mechanical Soft Texture foods require gravy?
Yes. All Mechanical soft diets require some form of gravy to moisten food. Each menu should have a receipt we should follow. We don't choose what we want to moisten it with.
F584 Homelike Environment - Who is responsible for addressing odor throughout the building?
All Staff are responsible for eliminating odor. Housekeeping should clean room once daily and deep clean each room monthly. Between Clean Nursing staff is responsible for removing soiled items from room timely to prevent odors.
F588 Accommodations: Am I responsible for call lights off my unit?
Yes. Everyone is responsible for answering call lights. If you are at the nurses station you can answer the light on any hall that is sounding.
F684 Professional Standards - Should I administer Blood Pressure Medications no matter what at scheduled time?
All Blood Pressure Medication should have parameters. Medications should always be administered following 7 Rights: right patient, right drug, right dose, right time, right route, right reason and right documentation
F812 Kitchen Sanitation - Can Staff store food in the walk in refrigerator?
No. Only Resident food can be stored in kitchen. Refrigerator available in break room for staff food. No staff food in kitchen or in resident refrigerators at nurses stations.
F584 Homelike Environment - Who is responsible for picking up trash in hallways?
All Staff. Housekeeping staff should sweep all hallways and floors daily. All staff should pick up items seen between sweeping.
F695 Respiratory Care - Can I adjust O2 Setting?
No. Only LPNS and RNS are allowed to adjust O2 setting. If you see a resident's O2 Flow is higher than normal please notify your nurse to adjust as ordered.
F684 Quality of Care - What do I do if a Blood Glucose Check is 45?
Level 1 hypoglycemia (<70 mg/dl): Give the resident rapidly absorbed oral glucose (15-20g); Notify the provider immediately; Remain with the resident; Recheck blood glucose in 15 minutes: If blood glucose is 70-400, provide the resident with a meal or snack; If blood glucose is >400 (rebound hyperglycemia), administer diabetic medications as ordered and notify the physician; or If blood sugar remains <70 mg/dL repeat oral glucose and notify the physician for further orders.
F812 Kitchen Sanitation - Do we really need to take temperatures each day?
Yes. Temp and chemical logs are required to be filled out daily or Q shift. Logs must be completed per your managers direction.
F689 Accidents and Hazards - Who is responsible for picking up cigarette butts.
Houskeeping should clear patio areas once daily. All staff should be responsible to pick up any cigarette butts between cleans.
F925 Pest Control - Who should remove food and soiled items from resident rooms and dining room?
F686 Pressure Ulcers - If a Skin check is scheduled do I really have to do it?
Yes. Scheduled Assessments are not optional. Facility Policy is that Head to Toe skin check will be completed weekly. Failure to do so puts our residents at risk and every nurse is accountable for the skin checks on their shift. Any skin check not completed due to resident declining must be called to nurse supervisor prior to end of shift.
F812 Kitchen Sanitation - When do I need to wash my hands?
When I enter the kitchen, anytime I change my gloves, anytime they are soiled, anytime I touch any surface.
F584 Homelike Environment - Do I really need to remove curtains in resident rooms if they are soiled?
Yes. Anytime a curtain is soiled it must be removed and cleaned.
