What is the Emergency Card?

A form or document that contains vital information about a participant to be used in case of an emergency. 

It is a critical component of a day program's emergency preparedness plan. The purpose of the emergency card is to provide essential details to staff, first responders, and emergency contacts to ensure prompt and appropriate care in urgent situations.

What is the reporting procedure for Day Program? 

  • Staff must contact (speak to in person) the Day Program Manager by 6:30am when calling out for a shift.  The manager will arrange for coverage of transportation route (or cover route themselves when needed).
  • If the Day Program Manager cannot be reached, staff must contact (speak to in person) the Day Program Supervisor.
  • If the Day Program Manager and the Day Program Supervisor can’t be reached, staff must call the Day Program at 8am and inform the respective Manager of the call out.

What is the reporting procedure for Residential, timeframe to call and who do you call? 

Monday through Friday from 8:30am-4:30pm, contact the office duty phone 856-202-3722 and speak to Office Duty Personnel to make a report.

On weekdays, before 8:30am and/ or after 4:30pm, weekends and holidays, contact on-call as outlined below.

Call 856-994-8432 for On-call Phone 1 and 856-994-8433 for On-call Phone 2.

What is the process of AM transport from arriving to the program to pulling out of the driveway?

Clock in, go inside to get the keys, warm up the vehicle, complete van cleaning and vehicle checks, assist with loading individuals on van, ensure each individual has lunch/money for lunch and adaptive equipment, ensure all are buckle and its safe to proceed on the DP run.
