Check this box if the provider has knowledge of and/or experience that may help serve children with challenging behaviors, children who are experiencing emotional distress, and children who may have significant delays in social skills development.

Check this box if the provider offers a preschool curriculum for preschool-aged children (ages 3-5), whether or not they operate a part-time preschool program. See “Type of Care” for clarification.

Check this box if the provider is willing to care for children who do not regularly come to child care, but who require care on a more ad hoc or drop-in basis.

Programs that run part-day and/or part-week programs, have an educational (i.e. preschool) curriculum, and almost exclusively care for children of preschool age (3-5 years). Preschool programs are legally exempt from certification by the ELD, although some may elect to become licensed as Certified Centers or recorded with ELD as a PS program. Programs that function as a preschool should be marked as Preschool Program, even if they are licensed by ELD. In general, if the provider accepts children of non-preschool ages or cares for some children on an extended basis, then the provider should be marked as a center, not as a preschool.

Check this box if the provider has made arrangements for a substitute provider in the event that they become sick or are otherwise temporarily unable to care for the children in care.

Check this box if the provider has knowledge/experience/ability to transfer children to/from wheelchair and/or to aid in the use of other assistive devices, such as walkers and crutches.

Check this box if the provider receives Teen Parent Program funding. This typically means that the provider cares exclusively or primarily for teen parents at the school and provides little or no care to the general public.

 Providers caring for children in their own homes. These providers maybe licensed with the ELD (Certified or Registered) or they are exempt from registration requirements (because they care for three or fewer children).

Check this box if the provider has a policy that the family must pay for the time they commit to bringing their child/children to child care, even if for any reason (illness of child, family vacation, etc.), a portion of that time is not actually used.

They may also run on a full-time basis during the summer. School age programs accept only children of school age. Children must be checked in and out of the program by an adult; they are not free to come and go as they please.

There are a number of child care centers that include a preschool that enrolls children who only attend the part-time preschool.
