If a teacher is not present in the room, what is the proper protocol for securing test materials?
What is must be under two locks? 


Most classrooms are not equipped to secure test materials under two locks.  Therefore,  a teacher or staff member should always be present and able to view materials at all time. If a teacher needs to leave the room, they should call Mrs. Trish Green so she can page Ms. Hagan or another staff member to be with the materials in the classroom while teacher is out briefly.

This is the order which should be followed for test administration?
What is in the order of the test booklet? This means all third graders should be taking the same portion of the test at the same time and must go in order of the subject areas in the booklet. Test administrators cannot jump around in the test booklet.

These resources are approved for all content areas of the K-Prep.
What are #2 pencils, blank or lined scratch paper, and erasers. Scratch paper must be turned into the BAC with all test materials and destroyed daily. Scratch paper may NOT be used for fifth grade on demand writing. All pre writing must be done in the test booklet. A writers reference sheet will be provided and must be returned.

Give two examples of extraordinary circumstances according to K-Prep?
Recent traumatic experience.  Ex. Child placed in protective custody during testing window._. Depending on the situation, these students may still be required to test. BAC will contact DAC who may communicate with the state.

Explain how to use a seating chart correctly for testing.  
One seating chart can be used for the entire testing window.  Draw the arrangement of your classroom and write students' names to indicate their seat for entire testing window.  Put date/dates students are absent by student's name to be used for makeup testing.  You can use a separate sheet to draw your room arrangement and staple it to the main form.

This must come between every test section (even when testing a student individually).
What is a break? It can be a short break (10 minutes or less).  During breaks longer than 10 minutes or when all individuals leave the room, materials must be secured by the BAC first. (ie: Materials must be checked in before going to lunch.)

What are approved calculators, protractors or angle rulers, KPrep provided rulers. Calculators must be put away for the "no calculator" portions of the test. The rulers must be returned to the BAC.

What are students who have extended time listed in their current IEP or PSP or 504?  They can receive double time or time and a half, and it will be indicated on the current plan.

A student gets a nose bleed or vomits on a test booklet. What is the procedure?

Answers must be transferred to another test booklet (of matching form). For your own protection, let the BAC transfer all answers.   A witness must be involved with all answer transfers. The contaminated booklet must be saved, then securely destroyed once the DAC has given permission. Do not do this yourself or even with another teacher present. Do not destroy any test materials. Let the DAC handle that.

All individuals involved (staff, students, parents, etc.) Content area and test part affected (Math, part 1 B) Specific content questions (if appropriate--do NOT include test questions in report, just the numbers!!!) Record what you learned 4 documents: Testing allegations reporting form Notarized document on school letter head, written and signed by each individual involved (1 form with multiple signatures is NOT acceptable.) Testing Schedule Seating Chart with Room Numbers (BAC must provide additional documents including: signed admin codes and proper test administration forms for proctor, non disclosure agreement forms signed by all involved, student name and ID, etc.)

Give 3 examples of when a student can participate in make up exams.
What is Once the child has returned to school and a new testing section has begun (ie: a third grader is tardy to school. She arrives during Math, part A. She can join in with her class during Math, part B. She will make up Math, part A in the afternoon.) 

During one of the scheduled make up days following testing.  This is for students who miss several days of testing due to illness, etc. 

 The recommendation is for a child to make up the missed portion of the test as soon as possible.
