What is the CN doc tool?
The platform Care Navigation can use to schedule HC visits.
What is PCSS?
The area that will identify if a member is engaged or non-engaged with Landmark Health.
What is the Primary Service in the PW banner?
This tab is located in PW where you would document resources given to a member.
What is the SDoH tab?
Care Navigators are prompted to do this when you cannot schedule in PCSS due to: no availability, member is ineligible, requests a virtual visit or if an interpreter is needed.
What is warm transfer to HCs?
Less populated or rural areas that Landmark Health will no longer be able to service.
What is low density?
When the Care Navigator is not in the scripter or selects a certain disposition and the scripter does not provide automatic documentation.
What is manual documentation?
Referrals made section selection in your PW communication note when you schedule a member in PCSS.
What is HC/Accepted - Direct scheduled?
A supplemental service in the DC market only that provides members with assistance with HHA assignment, HHA payments, provider referrals, etc.
What is LTSS high risk waiver?
These campaigns and/or dialer will not auto document for you.
What are Urgent & Outreach campaigns or precise/power dialer?
Is the amount of days Care Navigators can schedule members in PCSS in advance.
What is 60 days?
A supplemental service in which all clinical escalations will go directly to the members PCP.
