When speaking with NON-CLOSE gatekeepers who do not live with the Respondent.
What is never say the name of the study?
Say you are trying to reach the respondent. For example, if you visit a respondent who lives in a high-rise building with a doorman and the doorman will not grant you access to the building, you will need to find a way to get past this obstacle.
Even if the doorman claims to have a close relationship with the respondent, you should not disclose details of your visit, other than your credentials with NORC.

When will the respondent be home?
When is the best time to reach the respondent?
If you are told that a respondent is ill, on vacation, or away for other reasons, probe to determine the date on which the respondent is expected to return.
Use your best judgment of what to share (brochures, letters, dossier, etc.). This means it is ok to mention NSHAP. Often you may find yourself having to do gaining cooperation with the gatekeeper and the respondent simultaneously. This is acceptable and happens on NSHAP.

If the same phrase is repeated again and again, you can read it only as necessary or at specific intervals such as every fourth item in a row.

NSHAP is a nationwide study funded by US national institutes of health exploring how health, aging & social relationships are related.
What is the purpose of the study?
You do these tasks once a week. (multiple answers)

Send your FM the Yellow copy of the R fee receipt.

What is an F2 comment?
At the end of the interview, CAPI will prompt you to leave a hard copy questionnaire with the respondent.
What is the Leave-Behind Questionnaire or LBQ.
At the end of the interview, CAPI will prompt you to leave a hard copy questionnaire with the respondent. We will be referring to this document as the “Leave-Behind” Questionnaire (LBQ).
This questionnaire is to be completed by the respondent after you have left the household. The respondent will mail the completed questionnaire to NORC in a pre-addressed, postage-paid envelope. If the respondent provides an email address during the questionnaire, we will send them a link to complete an online version about a week after the interview.
You do this when No one is home/No answer at the door.
What is leave a SIMY card (use the Orange envelope)?
BONUS answers:
Ask the neighbor if they know if the R lives there or when they will be home.
Enter detailed call notes about what you saw so you know what changed the next visit.
Go back later in the day to see if the R is home or the SIMY is missing.

What is a signature?
The consent form is appropriately signed when a respondent has clearly chosen one option and initialed the box on the CO copy. Name is printed and signed; date is correctly filled out.
NSHAP Respondent fee receipt - BOTH boxes are checked, R has signed and dated, the FI enters their employee ID and name.
This questionnaire should be administered to all respondents after completing the main NSHAP survey.
What is the Results Questionnaire? It should be administered to all respondents after completing the main NSHAP survey.
You can access the Results Questionnaire through NORCSuite by clicking on the “Instrument” drop-down menu on the ‘Do Interview’ tab. During this section, you will provide respondents with the results of some of their physical measures (if they provided any).
Interviewers are required to do this at the door and/or in the home.

BONUS: in previous rounds, some respondents received $400 or $500. We should ask them to participate with an offer of $100.

BEGINNING OF INTERVIEW
Respondent is asked for verification to read the consent.
AFTER VERIFICATION TO READ CONSENT
Respondent reads (or has read to them) the Consent Form and signs before continuing interview. There is one version for Cohort 1 and one version for Cohort 2.
FUTURE ANALYSIS CONSENT (At end of Interview administration)
Respondent reads and signs the Future Analysis Consent Form.
GENETIC ANALYSIS CONSENT
For Cohort 2 respondents who provide blood samples only: the respondent reads and signs the Genetic Analysis Consent Form.
You do this at home after the interview was conducted.
What is the Interviewer Comments Questionnaire?
These items are to be completed by you after the interview has been completed.
You are to complete them at your own home the same day the interview was conducted. Interviewer Comments is a separate questionnaire and is not part of the main NSHAP survey.
It asks you to describe your experience interviewing the respondent and your perception of the respondent’s physical environment.
You can access the Interviewer Comments Questionnaire through NORCSuite by clicking on the “Instrument” drop-down menu on the ‘Do Interview’ tab.
