Who is responsible for training the residences? 

  • The Residence Nurse trains the residence staff on DNR policy/procedure in place when a person with a DNR is designated to the residence they manage.

Why is a reasonable accommodation important? 

Accommodations and modifications are designed to increase a supported person’s independence in his/her/their own home while maintaining his/her/their health and safety.  An accessible environment in the person’s home is important for his/her/their safety and well-being.

How does the policy define Lateness?

  1. A. Lateness - Definition:

An employee is considered to be late when he/she/they:

 

  • Fail to report for work at the assigned/scheduled shift start time without prior approval from the employee’s supervisor for the late arrival

     

    • There is no grace period for a late arrival for a shift

 

Excessive Lateness - Definition:

Excessive lateness is defined as two (2) or more occurrences in a two-week period regardless of whether the employee called in advance of the late arrival

 

Chronic Lateness

Chronic non-legally protected lateness may lead to disciplinary action up to and including termination

What does CPR stand for and what does it mean?

Any measures to restore cardiac function or pulmonary function in the event of a cardiac or respiratory arrest such as manual chest compression, mouth-to-mouth rescue breathing, intubation, direct cardiac injection, intravenous medications, electrical defibrillation, and open chest cardiac massage. For purposes of section 633.18 of this Part, cardiopulmonary resuscitation shall also include the transfer of a person to another facility if solely for the purpose of providing cardiopulmonary resuscitation.

How does the policy define earliness?

B.  Earliness - Definition:

An employee is considered to be excessively early when he/she/they:

  • Report for work more than 7 minutes before assigned/approved shift time starting without prior approval from the employee’s supervisor for the excessively early arrival

 

Excessive Earliness- Definition:

Excessive Earliness is defined as two (2) or more occurrences in a two-week period regardless of whether the employee called in advance of the early arrival

When is the first test and the second test be done?

  • The first test should be taken immediately for those who have symptoms. This is day 1.

  • The second test is taken ONLY if the first test is negative and should be done on day 3.

What is a DNR? Please provide the acronym and meaning.

 A legally binding order signed by a physician at a person’s request.  Its purpose is to provide Instructions not to attempt cardiopulmonary resuscitation in the event a person suffers cardiac or respiratory arrest. Such instructions may cover any cardiopulmonary resuscitation measures or may be limited depending on the scope of the consent.

What are the steps to address a reasonable accommodation?

  • Reviews the Request Form with the resident or his/her/their designee (if requested) to ensure the person clearly understands the form

  • Informs requester that the completed Request Form along with supporting documentation must be returned to the RM.

  • Once received, the RM will schedule a team meeting to discuss the requested accommodation

  • Advises requester of the procedure followed if the request is denied as stated above.

  • Provides process a requester may follow if he/she/they think a request has been denied unlawfully which includes filing a complaint by writing or calling the local/city/county human rights commission and/or either of the following:

How early does a supervisor need to be notified of a staffs absence prior to the start of the shift? How much time does the supervisor have to provide confirmation of notification?

  • Notification of at least four (4) hours prior to the start of their shift when the absence is reasonably foreseeable

  • Confirmation received from the supervisor within 20 minutes of notification.  If no confirmation is received from the supervisor, the employee must follow-up until confirmation is received. Supervisor is expected to respond within 20 minutes of receipt of message)

What are the steps for handling an emergency who has a DNR?


  • When a supported person who has a DNR experience cardiac arrest (abrupt loss of heart function, breathing and consciousness):

 

  1. Call 911 and report the person is experiencing any of these symptoms: the person is not breathing and the color is changing (staff should not perform CPR when there is a DNR order in place)

     

  2. Tell Emergency Medical Technicians (EMTs) as soon as they arrive that the person has a DNR

     

  3. Provide the signed DNR (or a copy) to the EMTs as soon as they arrive

When should a reasonable accommodation and/or reasonable modifications request for reviewed?

A completed Reasonable Accommodations and/or Reasonable Modifications Request Form should be reviewed: 1) whenever there is a significant change in the person’s medical condition, capacity or living situation, 2) upon request from the person supported and; 3) during the Life Plan Meeting (if necessary).
