Please refrain from utilizing your personal phone for any patient related activities. All contact to patients and resources should be done from an OH phone.

Please refer patients to these agencies together! This referral can be placed via email.
Who should be contacting the resource?
You should be contacting resources on behalf of patients to ensure that resources still have funding available as well as ensuring all information is accurate/up to date.
