What has you looking for a new primary care provider? What prompted you to look into our practice?
I want to see a MD
Our NPs are qualified and able to prescribe and diagnose as well. How has the care changed from seeing an MD to an NP?
We have a care team based approach which means our providers and health navigators communicate with each other and coordinate care together
I don't trust switching to a new doctor
That's a valid reason, trust takes time to build. Here at OMS we offer longer appointment times to ensure you are building a relationship with your provider. What's important that you find in a new pcp?
The VA handles my health care needs
We have a lot of patients who use One Medical Seniors in addition to going to the VA due to all of the additional benefits we have to offer, such as SD/ND visits and shorter wait times.

This is actually the best time to establish care, your care team gets to know your medical history before something urgent comes up. Once established we do offer SDND urgent visit when in need
