When should a task be created for a referral to Resource Coordination?
People who have previously been directly assisted by a resource coordinator should be referred to RTC for follow-up. If an RTC case exists, active or resolved, the resource coordinator has assisted the person in the past or is currently assisting the person and a task should be scheduled to the region of the NF. If the person is admitting to a SNF outside of MN, a task should be scheduled to the region of the previous assigned specialists.

If the nursing facility is different (transferred or admitted to a different one than what is on the PAS) and the PAS has already been processed, what needs to be updated?
-Update the NF listed on the PAS case in the CTS
-Make a note on of the call and the action taken on the timeline. Attach any documents that were updated
-Notify lead agencies of facility transfers IF they occur within 3 months of the PAS being submitted
-Update the NF on the PAS word document IF the person never admitted to the NF listed OR if we are notified within 3 months of when the PAS is submitted and an under 65 referral update is needed
What is the new process when a MAXIS case needs to be updated but MAP staff cannot make the update (the fields do not turn green)?
-In your PAS note in the CTS, include a description of the discrepancy and that MAP staff cannot update MAXIS. Also include who at the county was notified.
-On the PAS online form word template make a note of the discrepancy.
-Find the financial worker's information and leave them one voicemail detailing the discrepancy and how/who you confirmed the correction with (the submitter). If the phone number for the financial worker does not work call the number listed on the PAS contact list and provide them with the discrepancy. If the financial worker is from Hennepin county and the financial worker's phone number is listed as 612-596-1300 you can send the update via the exit document email address provided on the PAS contact list.

Who is responsible for adhering to PASRR?
Nursing facilities are responsible for adhering to PASRR, so compliance with PASRR is an issue of their licensure and this should be monitored by the state compliance entity.
