The communication plan is a required component of the backup plan and must be completed. The communication plan should include instructions on what a participant is to do in the event of non-provision of services. This plan should demonstrate the capability of providing emergency backup and relief services to cover the essential service needs for each participant within a reasonable time frame.

documentation and how often?

It is preferred that the Direct Care Professional include a daily narrative outlining the services provided during the shift and participant’s condition. However, at a minimum, a weekly narrative is required. The narrative validates that services were provided and gives the Direct Care Professional an opportunity to capture relevant information, such as a refusal of services.
