What is the communication board?
Quick team connection @ 7:05am / pm each shift. Time to share pertinent details and connect with any float staff.
What is shift huddle?
EVERY patient on the unit needs this each day, regardless of diagnosis or type of IV line.
What is a daily soap and water bath, linen change and mouth care.
This needs completed every quarter through an online module and manikin hands-on check-off.
What is mandatory RQI training?
Includes height, weight, blood pressure, heart rate, respiratory rate, temperature and O2 saturation.
What is vital signs completed on admission?
These areas need wiped down each shift. Example: bedside table, bedrails, countertop, computer stand, IV pump/pole.
What are high touch surfaces.
Can be scheduled through EPIC by the UC under "Unit Manager" for a specific date and time that works best for the patient, family and nursing staff. Needs done every 7 days for patients with extended stays.
What is UV light cleaning?
ALL diapered patients need this treatment completed and documented within EPIC with each diaper change.
What is diaper cream?
Includes acknowledge, introduce, duration, explanation and thank you.
What is AIDET?
Requires no shoes, minimal clothing and a dry diaper for infants.
What is the correct way to obtain a patient's weight?
Alone, on their back and in an empty crib.
What are the ABCs of safe sleep?
This includes removing linen, trash, toys, equipment and all PHI from the room (ex. drug card). Along with nurses opening the medication pass-through & discarding IV fluids/meds.

Place a barrier between self & patient when holding/feeding; wash patient's hands frequently; wash toys daily or if soiled/dropped on floor; limit caregivers; avoid crowds; and ensure excellent peri care.
What are ways to keep our neutropenic patients safe and free from infection? (neutropenic precautions)
Includes 1 RN and 1 other staff member visualizing and agreeing upon the patient's height & weight.
What is dual height and weight for all patients receiving chemotherapy?
Keeping the floor clear of clutter, checking the environment each time we enter the room, ambulating our patients with non-skid footwear and discouraging climbing are all examples of this.
What are ways we help our patients from falling?
Wearing double gloves, placing diaper in yellow container and using Splashblocker when flushing the toilet are all examples of this.
What are chemotherapy precautions?
ALL diapered patients receiving chemotherapy need this completed Q2 hours to protect their skin.
What is a diaper change?
Care provided and behavior observations or changes are documented every 15 minutes in electronic medical record by means of various electronic devices.
What is a responsibility of the constant attendant?
Needs completed on all patients less than or equal to 24 months of age and on all brain tumors upon admission and for outpatient visits.
What is head circumference?
Assisting the patient with ambulation and the use of their SCDs when ordered is an example of this.
What are ways we help our patient stay "blood clot" free?
Can I help you to the restroom? Can I bring you some fresh water or something to play with? are both examples of this.
What is purposeful hourly rounding?
Includes recording all meals, weighing diapers after zeroing the scale, and recording all emptied urine & stool (including the volume or x1 if unmeasurable).
What is recording patient's (input & output) I/O's?
This is an example: “social media can often be a trigger for emotional distress which is why we do not support it while you are here. If you need to call your parents, I can provide you a phone, but I do need to be present during the conversation.”
What is a "do say" statement to make while sitting as a constant attendant?
Ensure the cuff width is about 40% of the arm circumference midway between the elbow & shoulder taking up about 80-100% of this portion of the arm.
