Bipolar woman gives birth to a child w/ holosystolic murmur worse on inspiration.

What does the child have, most likely?

Ebstein's Anomaly

The tricuspid valve is located between the right atrium and right ventricle of the heart, and separates the two chambers. In Ebstein's anomaly, the tricuspid valve forms too far down in the heart into the ventricle, causing it to not close properly. Can be caused by a mom who takes Lithium for BPD.

A holosystolic murmur, also called a pansystolic murmur, is a heart sound heard through a stethoscope that occurs throughout the entire systolic phase of the cardiac cycle, meaning it starts right after the first heart sound (S1) and continues until the second heart sound (S2), indicating turbulent blood flow caused by a faulty heart valve

Inhalation increases venous return to the R side of the heart (this is where the TV is) so you can hear how badly that valve is working on inspiration
