You can only use electronics that your teacher has okayed. No phones, headphones, or other electronics used in class except for a Chromebook when needed.

For a medical excuse lasting more than 2 days, what documentation is required and where must it be submitted?
A doctor's note is required; submit to the main office and provide a copy to the PE teacher.

It depends on what you are wearing. It is the teachers decision to determine whether the outfit is suitable for the activity of the day.

30 minutes of continuous activity (outside of any school activity) with sentences explaining the activity you did and parent confirmation

they expose any part of the foot, they do not have a structured sole, they do not have a back (exposed heel), they do not lace up, boots, platform soles
