Two consecutive years of service
Any employee who has not completed two consecutive employment years will forfeit all accrued PTO on their final day of employment.
Eligible employees who leave in good standing will receive a final check of accrued PTO based on the PTO payout scale. (2: 25%, 3: 50%, 4: 75, 5+: 100)
Can employees post anything with patients, vendors, or other employees on their personal social media accounts?
Not without written consent of the patient and not without approval from Administration due to privacy rights.
How wide does a top strap need to be in order to be considered appropriate?
Tops with straps must be at least two fingers wide to be considered appropriate.

How many points is tardy and late counted as.
Tardy (when an employee is less than 15 minutes late for either their scheduled work time or when returning from a lunch break) : 3 Points
Late (when an employee is 15+ minutes late from either their scheduled work time or lunch break): 5 Points

How many hours of PSL is allowed to be used annually?
40 Hours.
When can a patient sign paperwork for themselves.
When 18+ they can sign for themselves.
If we are treating a patient and they turn 18 during treatment they must sign a NEW consent to treat form. They must also sign / give verbal consent to release information to parents after turning 18.
Are Spooner sweatshirts and hoodies considered appropriate?
No - Spooner hooded sweatshirts and hoodies are considered inappropriate.
Other inappropriate clothing includes: mini skirts shorter than where fingertips fall when arms are placed by the side of the legs, yoga pants, denim blue jeans, underwear revealed through outside of clothing, T-shirts other than Spooner branded, low cut revealing clothing, later tops, tube, tank, strapless, see through clothing, jerseys featuring sport teams, and any article of clothing with any logos, hats, flip flops/crocs, and camouflage

A new accrual type added to Paycom this year that is used when the supervisor has not been notified of the absence within two (2) hours prior to the start of the scheduled work time.
Employees must enter this on their time sheet.
What does it mean to be in Good Standing?
To qualify as being in good standing an employee must not have a positive discipline within the last 90 days.
How much notice is required for a PTO request to be considered and possibly approved?
2 Weeks.
Within 48 hours if the incident.
For employees, patients, or guests.
Are leggings allowed to be worn by all employees?
No - leggings are only allowed to employees that are not working on the patient care floor (PSC, and HQ employees)
Is a communication from a disciplinary action?
The communication form is not a discipline, rather it is a tool used to coach, address, or guide an employee on all sorts of issues that can arise during the work day.
An employees acknowledgement is NOT required to make the communication form valid.
What does an employee need in regards to their schedule to leave the company in good standing?
The employee must work their full schedule until the last day of employment.
How much PTO can be carried over to the next year?
Employees may bank up to one year worth of their annually / accrued PTO which will be eligible for carrying over into the following year.
Any PTO in excess of one year by the end of the last payroll cycle of the year will be forfeited.

Yes, supervisors reserve the right to bypass and/or repeat steps of the positive discipline process at their discretion, depending on the severity of the circumstances involved.
What is a voluntary resignation?
If an employee has two consecutive no call / no shows, it is considered voluntary resignation and they will not leave in good standing.
