Create a SNOW incident for lab customer support, include the reason you are canceling the specimen, create a SHIF including all the information that happened with the tube. Cancel tube with "5092- RECOLLECT-LAB ERROR ACCIDENT" and apply incident number in the pop up.

If a tube has parentheses around the location, what does this mean?

It means the tube has been removed from that location.

If you receive a partial barcode but only the bottom barcode is showing, what do you do?

Search the MPI and find the current requisition and label the sample as so. Scan the partial barcode in the spreadsheet.

If a specimen has been resulted and has already been relabeled, and is either in the HVS or on the automated line, what do you do?

It is a true duplicate, and must be documented.

For a mislabel, what needs to be included in the SHIF?

A picture where the patient name and entire tube is visible, all patient information of both the specimen and the incorrect label.
