What types of services does Personal Accident Insurance cover?

• ambulance (once per covered accident, per covered family member)
• initial services (received within one week of covered accident in a hospital, urgent care center or physician’s office)
• hospitalization, including intensive care unit (ICU)
• major diagnostic exams: CT, MRI, EEG, diagnostic X-ray (covered up to $750 per exam; one per covered accident*)
• tests and X-rays, other than diagnostic X-rays:
(one test or one set of X-rays within 90 days of covered accident)
• surgery (up to two per covered accident, per covered family member; performed within six months of covered accident)
• drugs (administered in a hospital, urgent care center or physician’s office at time of initial care of a covered accident)

What is the difference between an HMO, an EPO, and a PPO?  

HMO

  • HMOs offer coverage within a regional service area (5-county area + contiguous counties)
  • HMOs do not offer out of network coverage
  • On an HMO, you will select a primary care physician who will coordinate all your care and refer you to a specialist as needed
  • In order to see a specialist, you must get a referral on an HMO
  • Depending on the location, you may have access to Away From Home Coverage

EPO

  • EPOs offer coverage throughout the US with National Blue Card PPO
  • In an EPO, you do not need to select a primary care physician or need referrals
  • EPOs do not offer out of network coverage

PPO

  • PPOs offer coverage throughout the US with National Blue Card PPO
  • On a PPO, you do not need to select a primary care physician or need referrals
  • On a PPO, you can see a doctor that is out of network; however, there is a deductible that must be met before anything is paid

If I switch from Adult Premier Dental to Adult Preferred Dental (or vice versa), will my waiting periods start over?

Waiting periods are for new members or when there’s a break in coverage. If there is no break in coverage, waiting periods do not have to be met again.

What are the metallic levels and what do they mean?

Each plan is assigned a "metal level" based on the average percentage of healthcare paid by the plan. This makes it easier to compare similar health plans across health insurance companies. The metal levels are as follows:

Platinum: Your health plan pays 90% on average. You pay about 10%
     ○ Best when you plan to use a lot of healthcare services OR if you prefer the SECURITY of knowing you have the highest level of coverage
     ○ Higher monthly premium, lower out of pocket costs

Gold: Your health plan pays 80% on average. You pay about 20%
     ○ Best when you want to save on premiums and still keep out-of-pocket costs low

 

Silver: Your health plan pays 70% on average. You pay about 30%
     ○ Best when you want to balance your monthly premium without-of-pocket costs

Bronze: Your health plan pays 60% on average. You pay about 40%
     ○ Best when you don't plan to need a lot of healthcare services
     ○ Lower monthly premium, higher out of pocket costs
