What is an SV Intake Form
I am to provided assistance to a caller via a trained sexual violence staff or volunteer. I provide information about available services/resources, information about the effects of sexual violence and possible reactions and can give referrals to callers

I am there to help ensure survivors are informed of their rights and that those rights are upheld and to provide in person support to either Medical or Legal appointments
