An RA writes: “Resident Smith seemed intoxicated.” Rewrite the observation in the way the guidebook expects.

What is SIV Activity Log entries should be brief and should NOT contain names or specific details of the disclosure? The entry should document that a concern was disclosed, CSA responsibilities were activated, and the appropriate response occurred.
