You have been working with a patient for 6 months in which the patient and his parents seem to really trust and care for you. One day at pick up, the patient's Mom gives  you her phone number and asks if you would ever be interested in babysitting as her son loves you and asks for you all the time. You let the mom know that would be great and you feel honored they would ask!

Kim has been an RBT for 3 years and was a part of a team that started a new patient about a month ago. The patient has started engaging in aggressive behaviors, hitting and biting, when asked to terminate preferred activities or toys in which there is not a Behavior Intervention Plan for yet. The RBT placed the demand to put an item away to start transitions, the patient begins kicking the RBT. The RBT places the demand again to put the item away. The patient kicks the RBT again. The RBT blocks the patient's kick and then removes the item and puts the item away.

Code 2.05: Implement punishment procedures by direction of supervisor and behavior change plan

Removal of an item from a patients hand is considered a punishment procedure, this was not at the direction of the supervisor now was it a response in the BIP. The RBT should have re-established motivation and regained assent before representing the demand and if necessary by the skill set of the patient, engaged in prompting procedures to assist the patient in following the demand (model putting another item away, put the box the item goes in closer, etc).

The RBT could pack their own lunch everyday and eat that with the patient and respectfully decline the gesture as well as remind them of the RBT ethics code.
