What is "Conclusion of Intake" (within 60 days of the report)?
This parent must be included/rated on the formal Risk Assessment.
Who are the mother and father?
In the PA Model of Risk Assessment, the 15 risk factors fall into these 3 categories.
What are child factors, parent/caretaker/HHM/ perpetrator factors, and family environment factors?
This is the rating/judgement of the seriousness or degree of harm that has been experienced. For example, the judgement that death is more serious than a fractured leg is an overall judgement of this.
What is "Severity"?
This is the timeframe for completing a formal Risk Assessment when closing a case.
What is 30 calendar days prior to case closure?
When any child remains at home, a formal Risk Assessment is completed at least every 6 months in conjunction with these.
What are the FSP and/or judicial reviews?
This person who may have harmed the child must be included on the formal Risk Assessment.
Who is the (alleged) perpetrator?
Since they don't pose a risk of future harm to the child(ren), this parent does not need to be rated.
Who is a deceased parent?
This rating is a prediction of the likelihood of future child abuse or neglect without agency intervention based on the information gathered about the family.
What is "Overall Risk"?
If the county agency is unable to assess a particular risk factor (rating of "x" on the matrix) this must also be done when completing the Risk Assessment.
What is document the reasons it could not be assessed and the significance of the missing information?
A formal Risk Assessment does not have to be completed on a case if these one of these three circumstances exist.
What are 1) the children are in placement for 6 months or more and there are no children in the home, 2) the case has been accepted for services and remained at low or no risk, or 3) the referral is determined to be "bogus" (after one contact with the family the report is determined to be without any merit).
These people who have responsibility for the care of the child must also be on the formal Risk Assessment.
Who are the caregivers and the primary person responsible for the child's welfare?
Factors with these ratings can be family strengths and may be able to be used to mitigate risk.
What are "low" (L) or "no" (Z) ratings?
These are the two factors which are primarily considered when determining overall severity.
What are #2 (severity, frequency, and/or recentness of abuse/neglect) and #4 (extent of emotional harm)?
This is the first time an assessment of the risk to the child(ren) is documented based on initial information received.
What is at the time the report is received ("Risk Tag" at screening)?
When the agency is planning to return a child/children home, a formal Risk Assessment must be done at these times.
What is 30 calendar days prior to the child's planned return and again 30 calendar days following the return home?
These people must be included in the formal Risk Assessment, even if they don't provide care for the child(ren).
Who are household members?
An FSP should include goals/objectives designed to lower risk factors with these ratings.
What are "Moderate" and "High"?
These two factors are the starting place for rating overall risk.
What are "vulnerability" (#1) and "prior abuse/neglect" (#3)?
For the most accurate assessment of risk and safety, thorough information must be gathered in these 6 domains.
What are Type of Maltreatment, Nature of Maltreatment, Adult Functioning, Child Functioning, Parenting, & Discipline?
When a child/children's return home is unplanned, this is when a formal Risk Assessment must be completed.
What is within 2 weeks of the actual return to the home?
These children do not have to be included on a formal Risk Assessment.
Who are children in placement for 6 months when there are no other children in the home?
Factors which are high intensity, long duration, and environmentally supported should be carefully considered and generally have this impact on the overall risk rating.
What is raise the overall risk rating?
When determining Overall Risk, the "overall severity" rating is considered along with these.
What are the rest of the ratings?
This is a description of how all the risk factors interact in the family and impact the child(ren).
