Discussion with surgeon/proceduralist and one Banner clinical team member after the procedure to ensure correct procedure/site was performed, correct specimen, correct medium, correct patient label, post procedure instructions for the patient, and post procedure vital signs reviewed.
Who is responsible for the Debriefing?
Time outs must be done for separate procedures on the same consent.
What process needs to be done prior to proceeding with multiple safe procedures on the same patient?
Marking with an indelible marker with proceduralist initials or use of an anatomical diagram if unable to perform this step.
What is surgical site marking?
The Banner Clinical Employee (RN, LPN, or MA) that is in the procedure with the surgeon/proceduralist.
