What is PACE?
PACE stands for "Program of All-Inclusive Care for the Elderly". The goal of the program is to wrap you with services to keep you in your home.
Can I call my social worker if I am confused who I should speak with?
Absolutely - Everyone's assigned a social worker when you sign onto PACE. They will be your point of contact for non-medical issues.
Does everyone receive the exact same services?
No - All services are tailored to each participant.
Will I be in trouble for expressing a grievance/concern?
No - You always have the right to bring issues to our attention so we are able to address them.
Do all of my medical appointments need to be scheduled through PACE?
Is PACE available all day, every day?
If I need help finding housing, utilities or any other resources, can I call my social worker?
Yes - The social worker can point you in the right direction or assist you with applications.
Will PACE provide me with a maid?
No - If light housekeeping and/or laundry is a specific need, you will be sent a CNA who will "assist" you with these tasks.
How many days can be expected for a grievance to be resolved?
7 days
When you are feeling bad and think you need to call 911, what should I do?
Call PACE
Can my family members, friends, or pets ride on the bus with me?
No - Only participants are allowed on the bus due to liability.
When I'm in the hospital or rehab facility and I have questions, I would speak with my social worker.
False - We have a RN Case Manager who would follow you if you're in the hospital or if you've been transferred to a rehab facility.
Can my CNA pick up my mail or mail things for me or take my rent to the office.
No - The aides are not allowed to touch your mail or money/checks.
Is a request to increase, decrease or to provide a new service called a Give It To Me Request?
No - This is a Service Request.
If I have a non-urgent medical question, who do I call?
You would contact your PACE clinic nurse during normal business hours.
Can I eat or drink on the bus?
No - liquids and food are prohibited.
I can only see my social worker when I come to the clinic.
False - At times, your social worker may visit you in your home.
Can my CNA take me to the store or pick up fast food?
No - Your CNA can not take you anywhere or go to the store or gas station. All care must be provided in the home.
If your Service Request is denied, do you have the right to appeal?
Yes - We will provide you with the information on how to appeal.
How will my appointments with outside providers be scheduled?
The scheduler will schedule the appointments based on the urgency determined by the provider.
Will PACE provide me transportation to wherever I want to go?
No - the buses are used for medical and center transportation only.
I can talk to my social worker about anything and they won't share it with others.
Your personal information will never be shared with any other participant or their families. The social worker may need to share some information with your "team" of professionals in order to assist in your care.
How many hours of homecare will I receive?
The amount of services you receive is based on your specific needs.
Is "Definitely Recommend" the response we strive for during the Participant Satisfaction survey?
Yes - We always strive provide the best possible care we can and appreciate everyone spreading the word that you would "Definitely Recommend" PACE.
If I'm running low on medications, who do I call?
You would call the "refill line". Don't wait until you are out of medicine. Give us a week or so to get the refill to the pharmacy and have it delivered to you.
