  • The expanding haematoma forces the medial temporal lobe (uncus) through the tentorial notch causing uncal herniation compressing the ipsilateral oculomotor nerve (CN III).
    The superficial parasympathetic fibres fail first, so the ipsilateral pupil becomes fixed and dilated (mydriasis, unreactive to light).
  • Progression adds ptosis and a “down-and-out” eye from somatic CN III palsy.
