When completing an observation session and form, please ensure you are completing the following steps:
1) Make a copy of the observation form to your personal drive from clinician resources -> annual review shared folder
2) Complete the form during the observation session on Google Docs
3) Review the form and feedback with your team member
4) Request the team member to complete the competency section of the form and sign the bottom (typing their name is okay or using Adobe)
5) Email the completed form to your RCM, your team member and payroll
6) Your team member should be coached to upload the completed observation form to paycom after they have signed

False! Documentation of caregiver education is required in each note and should be individualized and specific to the session. Even if you are working on similar things between sessions, the education should not be exactly the same

There are a couple different ways to complete this process.
1) Take screenshots of each certificate and upload into one Google document, which can then be uploaded to Paycom
2) If courses were completed on ContinuED, you can export a copy of your transcripts into a PDF and upload the PDF to Paycom
3) You can upload them using a .zip file
