  • Double checking HMIS that they are not active in another program

  • Ensure HMIS profiles exist for all HH members

  • Updating address, phone number, and email on their SF contact 

  • Update “Income from the last 30 days” section of the FA App using the verified income for the household. This amount should match the HGIVF. 

  • Checking Confidentiality Agreement, Terms of Assistance and Grievance Procedure form is filled out correctly
