There should be as few limits as possible. Limits are usually only set around safety issues-is what the child is doing unsafe for them, the therapist, or the toys (break). ACT-Acknowledge the feeling, Communicate the limit, Target acceptable alternatives
If your client says they've already been diagnosed before, do you include that in your diagnosis?
It depends. If they are currently taking medication related to it, mostly likely. If not...take your time assessing and deciding if the diagnosis seems appropriate. You can always note in Assessment that client indicated this information

Why would you not have a child clean up the playroom at the end of session and what can you do if a child is especially messy?
Some children need to be messy to fully express themselves. It can often show the chaos in a child’s life. Children need the toys and materials to fully express themselves, so in asking them to clean up we are asking them to clean up what has been expressed. It may inhibit a child's play and expression. Shorten the session for extra time to clean up if needed.
