A 26 y/o G2P2 comes in for her post partum visit and she and her husband are giggling and smell like they smoked the morning away. She asks what the scientists say about breastfeeding after smoking. She has heard breast is best and will also help her shed the baby weight but asks you what effect her THC use has on her bundle of joy.

Limited human data suggest that THC also appears in breast milk. There are insufficient data to evaluate the effects of marijuana use on infants during lactation and breastfeeding, and in the absence of such data, marijuana use is discouraged. Breastfeeding women should be informed that the potential risks of exposure to marijuana metabolites are unknown and should be encouraged to discontinue marijuana use.

Is medical marijuana real?

No (per acog)

Because marijuana is neither regulated nor evaluated by the U.S. Food and Drug Administration, there are no approved indications, contraindications, safety precautions, or recommendations regarding its use during pregnancy and lactation. Likewise, there are no standardized formulations, dosages, or delivery systems. Smoking, the most common route of administration of THC, cannot be medically condoned during pregnancy and lactation. Therefore, obstetrician–gynecologists should be discouraged from prescribing or suggesting the use of marijuana for medicinal purposes during the period before pregnancy, and during pregnancy and lactation.

A 19 y/o G1P0 is at her new OB visit with you. You ask her if she drinks, smokes or does drugs. She asks if she will get in trouble with the law if she tells you about what might be in her dirty sprites.

Per ACOG patients should be informed that the purpose of screening is to allow treatment of the woman’s substance use, not to punish or prosecute her; however, patients should also be informed of the potential ramifications of a positive screen result, including any mandatory reporting requirements. Seeking obstetric–gynecologic care should not expose a woman to criminal or civil penalties for marijuana use, such as incarceration, involuntary commitment, loss of custody of her children, or loss of housing

Describe Cannabinoid hyperemesis syndrome.

Cannabinoid hyperemesis syndrome is a well-defined but apparently relatively rare syndrome involving episodic severe nausea and vomiting and abdominal pain which is relieved by exposure to hot water (shower or bath) [94-96]. Topical capsaicin has shown some benefit, but standard antiemetics and antidopamine agents are of little or no value [97]. The pathophysiology remains unknown, but patients are almost always daily cannabis users for at least one year and symptoms resolve within one to two days of cessation of cannabis use. Diagnosis is usually made after symptom resolution with cessation of cannabinoids after other causes of n/v are ruled out. (UTD)
