This would be a great place to refer someone who is having difficulty finding a mental health provider in their area and is struggling with depression.
What is Behavioral Health?
Considered best practice to complete this when completing a PDC call and the member had an admission for a new/different or existing condition that caused a decline in a members mental, social or physical functioning.
What is a new HRA?
Streamlined benefits quoting tool used by associates to provide accurate, personalized benefits for each member and their plan.
What is DEBUT?
Benefit for member to receive assistance with completing activities of daily living (ADL) in their home if a member is receiving assistance with at least one ADL. Allows caregiver to take a break while the member continues to get care in a safe environment
What is Personal Home Care Services (PHCS)?
This must be completed with each PDC member who has a High or High CCM LOI.
This is where a member can receive 90-day supplies of their medications mailed to them and often at a reduced cost.
What is Centerwell Pharmacy?
If the member had this status prior to the current HRA completion, follow-up from the previous care plan notes must be completed.

What is mentor?
Provides 10 free frozen meals (inpatient benefit) or 20 free frozen meals (chronic condition benefit).
What is Well Dine meal program (Mom's Meals)?
This is the area where medications are updated during each MOC call that includes name, dosage, and instructions.
What is OML?
This is a group of specialists who are able to assist members with resources for financial and other needs. They are considered the experts with referring through HCN!
What is Support SS?
When documenting a PDC this must be included in the member action plan in the care plan.
What is 1 active intervention related to PDC?
This is a place where I can find resources and information for myself as an associate.
What is the Associate Support Center (go/asc) or my leader?
Complete eligible healthy activities, like preventive screenings and exams, and get rewarded with this program.
What is Go365?
This is where we must document any discrepancies regarding a medication where it appears as though the member is not taking medication as prescribed.
This group is able to call members in all SNP states. A referral should only be placed for High or High CCM members who have a medical need and could benefit from a consult for nutritional recommendations.
What is a dietitian?
No area of focus may be left in this state for a complete/compliant ICP update.
What is draft status?
This is where I find VCC form information for C-SNP members.
What is go/sro (service read only)?
Mail delivery benefit for eligible members who are due for colon cancer screening or A1c testing.
What are STARS Test Kits?
This area shows medications which are no longer "active" according to fill date and dosing instructions.
What is AMP? (Will also accept "What is Outcomes tab?")
This is where you would refer a member who is interested in obtaining a continuous glucose monitoring device (CGM).
What is CCS Medical Supply?
This area must be updated with each MOC call but isn't necessary for a SNP Support contact.

This is where you can see if the member is enrolled in Intermountain Healthcare IPA and if referral for BH is needed must be sent to NV Behavioral Health.
