The form documents the 5 W's of the incident. Ultimately leading to who is at financial responsibility for damaged or lost items and equipment.

An award is a medal, badge, ribbon, or appurtenance bestowed on an individual or unit. A decoration is an award to an individual for a specific act of gallantry or service.
What is a MIST report? What does "MIST" stand for?
Supplementing report to a 9-line given that has further medication information. It's implemented in the DD Form 1380 (TCCC Card). Mechanism of Injury, Injuries, Signs/Symptoms, and Treatment.
